00 an hour We are looking for an On-Call / Weekend Registered Nurse Case Manager for Price - Carbon County! Skills:
1.
Proficient in Hospice Nursing Skills.
2.
Excellence in assessment and communication skills.
3.
Organizational skills to adequately coordinate delivery of care.
4.
Able to effectively function without close supervision.
Education/
Experience:
1.
Current Utah RN license; 2.
Current Utah drivers license; 3.
Automobile Insurance on own vehicle; 4.
One (1) year acute care experience.
5.
Some home health/hospice experienced preferred.
Responsibilities:
1.
On-call for Hospice and VNA patients:
2.
Return all call within 30 minutes.
a.
Assess and problem-solve over the phone.
b.
Make visit if necessary of requested by patient or family.
c.
Inform case manager of call and problem resolution.
d.
Call MD if necessary; inform case manager or appropriate personnel as needed.
e.
Write verbal orders.
f.
Call DME and pharmacy for medication or supplies as needed.
3.
Admits:
a.
Responsible for admits during designated work hours when necessary.
b.
Coordinate with pharmacy, DME and case manager.
c.
Responsible for calling pharmacy for drugs needed during the night or on the weekend.
e.
Fill out a home health aide care plan and inform the Team Assistant.
4.
Attend hospice team meeting and mandatory in-services.
5.
Attend pertinent workshops, in-services, and agency meetings.
6.
Document and function according to Community Nursing Service and CNS Hospice policies and procedures.
7.
Turn in time sheets with progress notes per CNS policy.
8.
Call status changes to the status change hot line.
9.
Each employee is responsible for excellent customer service.
Our customers include at least current and potential patients and their families, physicians, discharge planners, payers, and equally important, those with which we work at CNS.
Direct care, coordination of assigned palliative, hospice and VNA clients.
Admission of new home care clients.
Facilitate communication with patients, family, physician and case manager.
